However, if you have only just started thinking about this, you may find it helpful to read the section on\u00a0<a title=\"\" href=\"https:\/\/www.heartfailurematters.org\/living-with-heart-failure\/planning-for-the-end-of-life\/\">planning for the end of life<\/a>, so you are more informed when you talk to your family and doctor.<\/p>\n<p><strong>How do I prepare an\u00a0<strong><a class=\"addtooltip\" title=\"A document that details the treatments and procedures you want or don't want to be considered at the end of your life. \"><strong title=\"\">advance care directive<\/strong><\/a><\/strong>? <\/strong><br \/>\nYour doctor\/nurse or support group will be able to guide you when preparing your advance care directive.<\/p>\n<p><strong>How do I prepare a\u00a0<strong><a class=\"addtooltip\" title=\"A document for your healthcare professionals that states you do not wish to be resuscitated if your heart and breathing stop. \"><strong title=\"\">do-not-resuscitate order<\/strong><\/a><\/strong>? <\/strong><br \/>\nThe decision to have a do-not-resuscitate order is made by you or your family in consultation with your doctor. Your doctor will explain exactly what this order means and will make sure you are fully aware of the consequences before you make this decision.<\/p>\n<p>Patients with an<strong>\u00a0<\/strong>ICD\u00a0may need to discuss this with their doctor.<\/p>\n<p><strong>What can I do now to prepare for the future? <\/strong><br \/>\nThere are many ways in which you can prepare for the future. Your doctor\/nurse or support group will be able to give you information about living wills, do-not-resuscitate orders, advance care directives and financial planning.
